Information Needed For Our First Meeting

The following illustrates the types of documents I may ask for. Its helpful to gather them before our meeting so you will be able to refer to them during our fact finding session. Be assured they will be treated confidentially and will be returned when the process is completed.  From your:

Personal Files:

  • Income tax return
  • Wills
  • Mortgage or other loan documents
  • Trust Agreements
  • Social Security statement

Employer Or Former Employer:

  • Payroll statement
  • Employee benefit booklet
  • Pension plans
  • Retirement savings plans (eg. 401k Statement)


  • Checking and Savings  accounts statements
  • CD statements
  • Credit card statements

Brokerage and/or mutual fund company:

  • Monthly statement
  • IRA statements

Insurance Company:

  • Life Insurance policy information
  • Disability Policy information
  • Long Term Care policy information
  • Health and medical information
  • Annuity statements
